Based on Busari (2017), the interprofessional collaboration between physicians and other health care team members is important because it has increased the collective awareness of each others’ knowledge and skills and contributes to the quality of care through the persisted improvement in decision-making.What is your responsibility as an AGACNP in the outpatient setting?Palliative care is a relatively new function for AGACNPs, either as part of an inpatient team or outpatient. AGACNPs on palliative care services manage patients with life-threatening illnesses; treat symptoms such as pain, dyspnea, or nausea; lead goals of care dialogues; and coordinate complex care by collaborating with other associates of the patient’s healthcare team (Elgart, 2019).How does it differ from an inpatient setting?Based on Elgart (2019), the National Organization of Nurse Practitioner Faculties describes AGACNPs’ role as providers who manage patients with unstable chronic, complex acute, and critical conditions. This broad definition means that the AGACNP role is not confined to the hospital; instead, it is defined by patient need. AGACNPs are qualified to diagnose and manage patients with acute or critical issues or acute exacerbations of chronic health disorders. Notably, they are qualified to obtain health histories, conduct physical assessments, formulate differential diagnoses, order and interpret diagnostic studies, determine management agendas, order pharmacology treatments and other therapeutic interventions, collaborate and disseminate with other associates of the healthcare team, and promote transitions across different levels of care.Discuss a situation in which you have successfully collaborated or delegated to ensure the patient’s plan of care goes as planned.I recall this incident during my clinical rotation last semester, and I was assigned to a patient that came in with a chief complaint of abdominal distention, pain, and nausea. The patient has ascites from chronic liver disease, and paracentesis was performed in the ED before moving the patient to the medical unit. I completed a thorough history and PE on this patient. One pertinent positive subjective finding I came across was that the patient had “chest-pain type” of discomfort on inspiration when he coughs. I made my preceptor aware of this assessment finding, and he asked me what I should order to investigate that. I suggested STAT Chest X-Ray, an EKG, and trops. EKG and trops were insignificant but the CXR report came back and revealed bilateral infiltrates, suggestive of Pneumonia.The patient needs may be emotional, spiritual, or physical in nature. Include discussion of how the Christian worldview perspective can be applied to meet these patient needs.Scholars have described nursing as a caring discipline (Elk et al., 2022). Elk et al. (2022) also explain that the WHO determined that a person’s health care needs include physical, mental, social, and spiritual well-being. Investigations have found a strong positive relationship between spirituality/religiosity and health and well-being.
